​As the parent or legal guardian of the listed above Delegate , I hereby consent to the named person(s) participating in the pageant offered by Girls With Goals, LLC. I further acknowledge, understand, appreciate, and agree that my participation may result in possible exposure to an illness from infectious diseases, including but not limited to, MRSA, Influenza, and Covid-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others and assume full responsibility for my participation and exposure. I UNDERSTAND AND ACCEPT THAT RISK. I have additionally communicated these risks to my child(ren) participant(s). I further understand that while the payment of fees constitutes a part of the consideration due to Girls With Goals, LLC for allowing my child(ren) to participate in the pageant, an additional and important part of the consideration is this signed release form. Therefore, in consideration for allowing my child(ren) to participate, I hereby forever release Girls With Goals, LLC, its owners, employees, and committee members from all liability for any and all damages, injuries suffered or illnesses acquired by my child(ren) while under instruction, supervision, or control of Girls With Goals, LLC, its owners, employees, and or committee members. As the parent or legal guardian of the aforementioned person(s), I hereby agree to individually protect for the possible future medical expenses which may be incurred by my child(ren) as a result of any injury or illnesses occurred while under the direction of Girls With Goals, LLC. I acknowledge the risk and waiver of liability, having been read thoroughly and understood completely, is signed voluntarily as to its content and intent.
